Nachdem der Käpt´n am Samstag einen Anfall hatte, scheint es seit Montag (10. Juli 2023) langsam bergauf zu gehen, der Muskeltonus und die Wachheit kehren zurück. Der Käpt´n kommt nun auf ca. drei “gute” Stunden vor der Tablettengabe, die exakt alle 12 Stunden erfolgen muss. “Gut” ist jedoch relativ zu sehen: Er steht in diesen Stunden nun vermehrt, fängt an zu betteln und nimmt zumindest einen Teil seiner Mahlzeiten im Stehen ein. Zwar sackt er währenddessen immer noch zusammen und auf die Seite, aber insgesamt kann er sich besser auf den Pfoten halten. In dieser Zeit kann er auch das Beinchen heben und kleine Gassirunden von 10 bis 15 Minuten (mit Pausen!) sind okay. Auch die Selbstwirksamkeit kehrt zurück: Er sucht sich selbstständig seinen Wurm zum Nuckeln.
Bei generalisierten Anfällen wirken unglaubliche Kräfte auf die Muskeln ein. Sie werden derartig angespannt, dass sogar Knochenbrüche verursacht werden können und starke Verspannungen zurückbleiben. Frau Loh von der Tierarztpraxis auf Gut Windeby hat uns regelmäßige Massagen und Osteopathie empfohlen. Letzteres können wir noch nicht umsetzen, aber mit den Massagen haben wir angefangen. Dazu nutzen wir das Novafon*. Ich füge nun noch eine Pfotenmassage und weitere physiotherapeutische Übungen hinzu, die bei Ataxie helfen. Morgen reduziere ich das bisherige Medikament um ein Sechstel. Mal sehen, was das bewirkt. In anderthalb Wochen wird der Phenobarbitalspiegel im Blut kontrolliert, dann müssen wir mit der Dosis sicherlich noch einmal hoch.
Es geht also – vorerst und in Schneckenschritten – bergauf. Ich bin dafür sehr dankbar. Der Kontrollverlust über den eigenen Körper, den ich ihm ja nicht erklären kann, muss traumatisch gewesen sein.

Phenobarbital Day 9 to 14
After Captain had a seizure on Saturday, it seems that things have been gradually improving since Monday (July 10, 2023). Muscle tone and alertness are returning. The Captain now has approximately three ‘good’ hours before the next dose of medication, which must be given precisely every 12 hours. However, ‘good’ is relative: during these hours, he stands more often, starts begging (for example, when I’m eating a cinnamon roll), and at least consumes part of his meals while standing. Although he still collapses and falls to the side during this time, overall he can better support himself on his paws. He can also lift his leg, and short walks of 10 to 15 minutes (with breaks!) are okay.
His self-efficacy is returning as well: he independently seeks out his worm to suck on (Picture 2). During generalized seizures, incredible forces act upon the muscles. They become so tense that even bone fractures can occur, leaving behind strong tension. Ms. Loh from Gut Windeby has recommended regular massages and osteopathy. We cannot implement the latter yet, but we have started with the massages using my novafon*.
Now I am adding a paw massage, which is supposed to help with ataxia. Tomorrow, I will reduce the current medication by one-sixth. Let’s see what effect that has. In a week and a half, the phenobarbital level in the blood will be checked, and we will probably need to increase the dosage again. So, things are improving – for now and at a snail’s pace.
I am very grateful for that. The loss of control over one’s own body, which I cannot explain to him, must have been traumatic.”

Día 9 al 14 de Fenobarbital.
Después de que el Capitán tuvo una convulsión el sábado, parece que desde el lunes (10 de julio de 2023) las cosas están mejorando gradualmente. El tono muscular y el ánimo están volviendo. Ahora el Capitán tiene aproximadamente tres horas ‘buenas’ antes de la próxima dosis de medicamento, que debe administrarse exactamente cada 12 horas. Sin embargo, ‘bueno’ es relativo: durante estas horas, se pone de pie con más frecuencia, comienza a suplicar (por ejemplo, cuando estoy comiendo un rollo de canela) y al menos consume parte de sus comidas de pie. Aunque aún colapsa y cae hacia un lado durante este tiempo, en general puede mantenerse mejor sobre sus patas. Durante este tiempo también puede levantar su patita y caminatas cortas de 10 a 15 minutos (¡con pausas!) están bien. También está recuperando su autonomía: busca su peluche para chupar.
Durante las convulsiones generalizadas, se ejercen fuerzas increíbles sobre los músculos. Se tensan tanto que incluso pueden ocurrir fracturas óseas y quedan fuertes tensiones. La Sra. Loh de Gut Windeby nos ha recomendado masajes regulares y osteopatía. Aún no podemos implementar esto último, pero hemos comenzado con los masajes usando mi Novafon*. Añado un masaje a las patas, que se supone que ayuda con la ataxia.
Mañana reduciré imepitoin, el medicamento actual, por un sexto. Veremos qué efecto tiene. En una semana y media se controlará el nivel de fenobarbital en la sangre y seguramente tendremos que aumentar la dosis nuevamente. Entonces, las cosas están mejorando, por ahora y a paso de tortuga.
Estoy muy agradecido por ello. La pérdida de control sobre su propio cuerpo, que no puedo explicarle, debe haber sido traumática.”
